Everyone is welcome to make a copy of a form for completion.

• Before starting to fill out any form, please call the Deputy Area Commissioner for Honours and Awards, Bruce Riddiough at (905) 549-3453. This will help ensure proper tracking of the write-up process.
  
  
• All completed forms must be sent to the Deputy Area Commissioner - Honours and Awards. Please leave them in the envelope at Scout House and follow up with a phone call to Bruce Riddiough at the above telephone number.
